GENERAL CONDITIONS
Please read carefully.
J. Martin's Heating & Cooling shall have the right to reject or discontinue this agreement if, on initial inspection, its authorized representatives deem the air conditioner or controls unsuitable for servicing, until the equipment is put into working condition. If service on any contracted Air Conditioning system is performed by anyone other than our authorized personnel, the contract will be immediately void without any reimbursement of any type.
J. Martin's Heating & Cooling will perform service as soon as possible under normal conditions. We shall not be liable for and delay or failure to supply such service or material, when such delay or failure is due to conditions beyond our control. In no event shall we be liable for consequential damage or injury.
Repairs or replacements necessary because of an "ACT OF GOD", flooding in or around unit, water leaking onto controls, sewer back-up, inadequate electric or air supply, or any other abnormal cause are not covered under service agreement.
J. Martin's Heating & Cooling will accept telephone calls for air conditioning service during normal working hours only. "Normal Working Hours" is defined as Monday through Friday, except holidays, 8:00 AM to 5:00 PM, and Saturday with extended contract 8:00 AM to 12 NOON. Any and all after hours or holiday service call will be billed at our over time rate.
Parts and labor not covered by our extended service contract will be paid C.O.D. upon completion of service. Parts and labor not covered by this extended contract are: cabinets, ductwork, fan assemblies and bearings, blower assemblies and fan blades, all compressors not covered by manufacturer's warranty, evaporator and condenser coils, line sets, outdoor condensing units (there will be a 10% discount for replacement of this unit), wiring from electrical panel to outdoor disconnect, low voltage wiring from indoor unit to outdoor contactor and air filters. To customers with JCP&L POWER SAVER control, J. Martin's Heating & Cooling will not assume responsibility for the operation of this control system.

I hereby agree to the conditions set forth in the above service contract and understand that this contract shall apply only to the air conditioning unit at the address listed below for one year form the date of issue.
